Senior Social Worker (PACT)
Social Work services include high risk screening, psychosocial assessment, consulting, managing crises, treating, referring, coordinating VA services and community resources and interfacing with outside health care systems. This level of interface represents a unique level of differentiation, complexity and interdependency of skills and tasks. Duties Include but are not limited to:
Works collaboratively with medicine and specialty clinics to further the VA mission in providing exceptional health care to improve the health and well-being of Veterans. Interviews Veterans and their family members/significant others to establish facts about their presenting problems, their causes, and the impact of such problems as it relates to their functioning and health as part of a comprehensive psychosocial assessment. Reviews all data, subjective and objective, and makes a clinical assessment, identifying needs and strengths. Utilizes advanced clinical training, insight and experience to interpret data and to identify viable treatment options. Assesses high risk factors, acuity and need for services to develop a treatment plan and objectives. Utilizes motivational interviewing and completes an individualized personalized healthcare plan. Performs specialized treatment of complex physical or mental illness. Incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges. Maintains the ability to provide individual, group, and family counseling and advanced level case management interventions used in the treatment of veterans. Develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for discharge planning. Works closely with the veteran or service member's interdisciplinary treatment team to assure good communication and treatment planning. Maintains responsibility for the screening, consultation, securing authorizations and admissions of high-risk Veterans. Functions as the primary contact for community agencies, hospitals, health care facilities, city and county governments, providing general information about VA benefits and eligibility. Coordinates and provides information in compliance with HIPAA and VA Privacy regulations regarding VA benefits pertaining to nursing home care, in-home assistance, and aide and attendance/housebound benefits. Evaluates Veterans' eligibility for VA nursing home care, and identifies the medical needs to distinguish between extended care services, other VA inpatient programs, long term care options, in-home health services and county services Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services. Demonstrates the ability to design changes based on empirical findings. Offers consultation to colleagues, social work students, residents, interns, and fellows from other disciplines regarding the psychosocial treatment of patients, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Demonstrates the ability to teach and mentor staff and students in the special area of practice and to provide supervision for licensure or specialty certifications. Provides ongoing collaboration, mentoring, and education to TXVCBHCS staff regarding the unique needs of the Veteran. Performs other related duties as assigned. Work Schedule:

Experience:
2 years of Medical Case management experience including experience in working with hospital setting, Home health, Skilled nursing facility and/or Hospice. Experience in working with high risk/specialty population such as Veterans, geriatrics, and patients with multi-comorbidities through clinical case management. Certification in Advanced Case Management or Certified Case Manager (CCM) preferred/if available. Must be familiar with local community agencies and resources. Grade Determinations:

The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification:
Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
