We are searching for Full-Time or Part Time Registered Nurse's to join our staff and deliver quality home care services.
Key responsibilities include:
Performs clinical assessments to identify patient needs and appropriateness of patients as evidenced by documentation, clinical record, case conference, team report, and evaluations.
Documents, prepares, revises as necessary, and completes a Plan of Care, patient notes, phone orders, and other clinical records documentation on an ongoing basis.
Supervise and provide clinical direction to the HHA/LVN to ensure quality and continuity of services provided.
Monitor assigned cases to ensure compliance with requirements of third party payor.
Uses effective interpersonal relations and communication skill.
Responsible for timely completion and synchronization of OASIS visits within patient software (Wellsky/Kinnser) and 485's.
We seek the following principal
Qualifications:
Graduate of an accredited school of professional nursing.
Minimum of 1 year of nursing experience in a HOME HEALTH setting within the last year.
Home Health and OASIS experience REQUIRED.
Current License as an RN in the state of Texas.
Travel is required; must have a reliable car w/current insurance coverage and a current driver's license.
Knowledge of Wellsky (Kinnser) Home Health Software preferred.
Current TB Test English/Spanish speaking required.
Current CPR certification.
Salary is dependent on experience.
