Summary:
We are seeking a highly skilled RN or LVN Home Health Nurse to provide high-quality patient care services in a home setting.
The Home Health Nurse will be responsible for assessing patients' health and providing medical treatment in accordance with physician's orders.
Additionally, the Home Health Nurse will be responsible for coordinating care with other healthcare professionals, including physicians, therapists, and social workers.
This is an LVN position with flexible scheduling, including weekends and holidays.
Responsibilities:
Provide skilled nursing care in accordance with physician's orders, including wound care, medication management, and patient education Monitor patients' progress and adjust the plan of care as needed Coordinate care with other healthcare professionals, including physicians, therapists, and social workers Communicate regularly with patients, families, and healthcare team members Maintain accurate and complete patient records Follow all regulatory requirements, including HIPAA and OSHA guidelines Adhere to agency policies and procedures Requirements:
Active and unrestricted RN or LVN license in the state of Texas.
Home health experience preferred Strong communication and interpersonal skills Ability to work independently and as part of a team Ability to adapt to changing patient needs and schedules Current CPR certification Physical Requirements:
Ability to lift and move up to 50 pounds Ability to stand, walk, and bend for extended periods Ability to drive to patient homes and other off-site locations as needed Salary:
Negotiable - Dependent on experience and qualifications Job Types:
