Enters data into various sources for processing and management.
Create spreadsheets and transfer sensitive or confidential data efficiently.
Retrieves data from a database or electronic files as requested.
Enters, manages, processes, tracks, and analyzes data for disease exposure events, follow-ups, and THISIS, an electronic disease surveillance system.
Carry out quality assurance for information entered into local and state surveillance systems, cohort review forms, and multi-drug resistant patient information sent to Heartland for a consult.
Communicate with local hospitals, private physicians, correctional facilities, and other public health entities to obtain medical information and ensure continuity of care.
Assist with conducting cohort reviews and updating monthly case summary forms for monthly case reviews.
Scan and upload documents into patient folders on the share drive; copy and file documentation into patient charts.
Collect data for use in planning and evaluating the effectiveness of public health education programs.
Confers with local health agencies, private physicians, individuals, and community action groups on the treatment, control, or prevention of communicable or chronic diseases.
Submits performance measures and other monthly and quarterly reports accurately and on time as requested by management.
